• Act 151 - Pennsylvania Child Abuse History Clearance  (1 Free Clearance Every 5 Years for Volunteers)
• Act 114: FBI  Criminal Clearance Report ($23.25) – COMPLETE *ONLY* IF YOU HAVE NOT LIVED IN PENNSYLVANIA FOR THE LAST 10 CONSECUTIVE YEARS.

  • Please upload the Unofficial Results or your receipt to Raptor Volunteer Management; an email will come to you from IdentoGO with this information.
  • Volunteer Affirmation Form - Those who have resided in the State of Pennsylvania for a period of 10 years or more at the time of registration do NOT need to obtain an FBI Criminal History Record check, but must instead download, sign, and submit this Volunteer Affirmation form with your online volunteer application.

PLEASE NOTE: Submitted clearances cannot be more than 1 year old from the date they were issued. DO NOT MAIL OR TAKE clearance request forms to any school. You will upload your clearances to our online application.

• A TB Test or TB Test Waiver is REQUIRED for all Position Volunteers

• TB Tests are available at doctor's office, a minute clinic, local pharmacy, or at the Methacton High School Nurse’s Office by appointment. To schedule a TB Test appointment with Methacton High School Nurse, email kthompson@methacton.org (you will be notified by email of a date/time/location)
• TB Test waiver may be submitted in lieu of a TB Test for certain Position Volunteers where volunteering is less than 10 hours per week.
